The “Confidential Litigant Contact Form for Kinship Legal Guardianship (KLG) Matters” is a two-page New Jersey Judiciary form, marked “Published: 08/2024 CN:13208” in the footer of each page alongside “page 1 of 2” and “page 2 of 2.” A notice at the top states: “This is not a public document. The information entered on this form will be kept confidential.”
That same notice identifies what the form treats as personal identifiers: “Social Security number, driver’s license number, vehicle plate number, insurance policy number, active financial account number, active credit card number, or military status.”
The form opens with a caption line reading “Kinship Matter of:” beside a “Docket Number” field whose printed prefix is “FL-.” The person completing it marks their own role under the heading “I am: (check one)” by checking one of three boxes: “Parent,” “Kinship Legal Guardian,” or “Other: (specify relationship).”
Three contact blocks follow, one for Parent 1, one for Parent 2, and one for the Kinship Legal Guardian. Each block collects a name, street address, city, state and Zip Code, a cell phone number, an “Other Phone” number, and an email address. Each block also carries a checkbox reading “I cannot provide the current address. I have done the following to locate this person:” followed by open space for a written explanation of the efforts made. The first two blocks and the beginning of the third sit on page one; the rest of the guardian block continues on page two, under a running head repeating the form’s full title.
After the three blocks, the form provides a section for “Other interested parties’ name(s), address(es), email and phone number(s).”
The document closes with a certification: “I certify that the foregoing statements made by me are true. I am aware if any of the foregoing statements made by me are willfully false; I am subject to punishment.” The form attributes that language to “Certification Rule 1:4-4(b).” Beneath it are a date line and a signature line labeled “Signature of Applicant,” printed after an “s/” marker. Page one carries “Print” and “Clear” controls. No filing fee, deadline or court address is printed anywhere on the form.
