“Order for Summary Jury Trial” is the heading printed on this New Jersey Judiciary form, which carries the form number CN 10878 and the notation “Revised 11/01/2013, CN 10878-English.” The document is two pages long.
The caption block at the top of page 1 identifies the court as the “Superior Court of New Jersey,” the “Law Division,” “Civil Part,” with blanks for the county and the docket number, and it labels the matter a “Civil Action.” A separate block at the top left is headed “Plaintiff or Filing Attorney Information” and provides spaces for a name, an “NJ Attorney ID Number,” an address, and a telephone number.
